If your child will be required to take any medicines whilst in school, please fill out this form.  Please note that the school will not be responsible for administering any medication without this form, and under no circumstances will the school administer Aspirin.  Please also note that painkillers such as Paracetamol are treated as medication.

St Chad’s takes seriously its duty under section 175 of the Education act 2002 to safeguard and promote the welfare of children and to work together with other agencies to ensure adequate arrangements are in place to identify, assess and support children who are at risk of abuse and neglect which may cause significant harm to their health and development.
